यो भरेर नेपाल प्रहरी वा नेपाल रेडक्रस मा लैजानुहोस्। तपाईंले सम्झनुभएका साना कुरा — दाँतको उपचार, पुरानो चोटको दाग, लगाएको गहना, हिँड्दाको लुगा — ले नाम वा फोटोले भन्दा बढी काम गर्छ। ६६९ शवमध्ये २० को मात्र पहिचान भइरहेको अवस्थामा यही विवरणले फरक पार्छ।

Fill this in and take it to Nepal Police or the Nepal Red Cross. The small things you remember — dental work, an old scar, the jewellery worn, the clothes last seen — do more than a name or a photograph. With only 20 of 669 bodies identified, this is the information that closes the gap.

The ruled lines below cannot be typed into, and nothing here is submitted online. If you cannot print, copying these headings onto paper in the same order works just as well.

This is not an official form. It is a preparation aid and does not replace an official report. The official channels are Nepal Police (100) and the Red Cross disaster hotline (1130).

प्रहरीले प्रत्येक शवको फोटो, DNA नमुना, ट्र्याकिङ कोड, शारीरिक विवरण, शव परीक्षण प्रतिवेदन र गाडिएको ठ्याक्कै ठाउँको अभिलेख राखेर मात्र गाड्ने गरेको छ। पछि आएर दाबी गर्दा पनि मिलान गर्न सकिन्छ।

Police record a photograph, DNA sample, tracking code, physical description, autopsy report and exact plot location for every body before it is buried. A family that comes forward later can still be matched.

Police are taking DNA from recovered bodies. Matching needs a family reference sample — a parent or a child gives the strongest comparison.
